To assist program customers working in production of light industrial assembly jobs while modeling appropriate behaviors, reinforcing behavior, and providing intervention as necessary. Essential duties and responsibilities include the following. Other duties may be assigned.  Assist customers in work training to increase vocational skills and improve work behaviors as directed.  Monitor program customers’ Individual Service Plan (ISP) objectives and assist customers in meeting work readiness goals with on-the-job training in areas such as: appropriate behaviors, safety standards, interpersonal skills, productivity, and work quality. Maintain production timesheets for customers on a daily basis, measuring hours worked, position and product produced.  Monitoring customer productivity levels and quality to meet the ISP objectives and payroll accuracy. Reports to the Senior Service Program Coordinator II and works with the Ohio Department of Medicaid HOME Choice Staff and the HOME Choice Case manager to successfully relocate an institution resident from the institution to the community. Contact nursing facilitates within our GESMV region and set up a time to perform outreach and education to the staff on the HOME Choice program.  The express objective of this outreach is to gain referrals to the HOME Choice program.  Work with the Goodwill Easter Seals Marketing and Communication staff in promoting service in the Community.  Work with the Nursing Facility (NF) staff in providing information on the HOME Choice program including the letter from ODJFS, a fact sheet and brochures.  Process ALL referrals from NF and assist resident and/or NF staff in filling out the HOME Choice application form, signed by the resident or the resident’s guardian.  The TC will then fax the form to the HOME Choice Central Intake Unit. Ability to manage budget.  Ability to think creatively.  Must be familiar with Resources, Housing and Benefits in the Community.  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
