The Recovery Navigator will be responsible for the initial contact with potential consumers seeking assistance.The Recovery Navigator will identify consumers’ needs by completing a thorough intake process.They will then assist in the navigation process by referring the consumer to appropriate services within the agency as well as, to outside agencies based on the consumer’s desired outcome.Requirements include:HS diploma or GED preferred, but will consider applicants working on their diploma.Preference will be given to candidates who are Certified Peer Specialist by the State of Ohio or in pursuit of this certification.Must be proficient in Word, Excel, Internet, Email and have previous customer service experience.Preference will be given to individuals with psychiatric disorders and those who have affiliations and/or experience with Clubhouse International standards and principles.

Program Services Social Worker would provide social work services as needed in various programs, including Behavioral Health, Child and Senior Services, DD Services and Workforce Development.  This person will provide social work/counseling services with referred Home Choice consumers according to ODM regulations for Home Choice Social Work/Counseling Services.  Coordinate with Home Choice Transition Coordinators and provide various social work functions in collaboration with a community partner seeking to link their clients with community resources.  This person will be responsible for completing behavioral health diagnostic assessments and treatment plans for Job Development case managers working with clients with mental health issues, as needed and assist participants and caregivers in making psychosocial adjustment to Adult Day Services program and provide on-going support, as needed. The Direct Support Specialist will provide home and community based services to individuals referred by contracting agencies according to their individualized service plans. Candidate will be responsible for the therapeutic interventions for children and families who are experiencing emotional /behavioral distress and/or disturbances.  Must be able to provide mental health assessments and develop treatment plans with the treatment team.  Be able to give direction to the entire treatment team and ensure the implementation of therapeutic activities and provide direct therapy for individuals, families and groups.

Must have strong communication skills and be able to effectively communicate with families, team members and the public all with various backgrounds.  Master’s degree in one of the following areas: counseling, social work, psychology, marriage and family therapy or a field directly related to children’s mental health counseling. Must be familiar with DSM IV and the Multi—Axis diagnostic system. Licensed or licensure eligible and have two years community mental health and/or professional experience working with children.

The Candidate will perform assigned duties including filing, typing and data entry. Provide support during open office hours as needed.  Route and file medical records, patient charts, and repo. Review patients’ charts to ensure proper documentation.  Maintain a professional office environment and promote a positive image for the clinic by acting and dressing appropriately. Assist support staff in the absence of another staff member or as necessity dictates; i.e. provide back up to receptionist.  Prepare correspondence, memos and reports in the proper format. Proofread and prepare documents for distribution.  Maintain Multiple Community Service Information Systems enrollment and coordinate that process with the Tri County Board as directed.  Receive and respond to inquiries of Releasing Medical Records as requested and approved by Clinical Staff per HIPAA and all state regulations.  Maintain chart information in Carelogic MIS.  Operate standard office equipment including, multi-line telephone system, postage meter, facsimile machine, computer/printer, photocopy machine.

At minimum, a high school diploma preferably post high school education.  Maintain up to date CPR, First Aid and TAPS certification.  Driver’s license (fewer than 6 points) and appropriate auto insurance maintained.  Must display good customer service skills with clients, internal colleagues and our community at large and utilize computer technology to perform clinical documentation in Carelogic (MIS System) directly, as well as Microsoft Office Products.
